Sky Quest, LLC takes responsibility for and has an obligation to protect the safety and wellbeing of each travel experience for our passengers, our employees, and our families. In support of this commitment, please answer the following questions to the best of your ability within 24 hours of departure. Crew members reserve the right to delay departure if they have not received completed forms or they suspect answers provided are false.

    2. Have you, in the preceding 14 days, had a fever (100.4 °F / 38 °C or higher), loss of taste or smell or tested positive for COVID-19?

    3. Have you, or any person in your party, had close contact with, or helped care for, anyone suspected or diagnosed as having COVID-19, or who is currently subject to quarantine for possible exposure to COVID-19?

    4. Are you physically able to ingress/egress our aircraft without assistance? This will require navigating 3-6 steps and walking a short distance?
